Qualifications Required

  • At least 1 year experience in long-term care and /or hospital setting
  • Latest unencumbered License in Washington State
  • Current CPR Card
  • Social Security Card
  • TB test results (yearly). If positive, please provide a copy of the chest x-ray (must renew every 10 years)
  • Hepatitis B Vaccination Record
